Chosen Name Change Request Form

Please scan the QR code or click HERE to access the Chosen Name Change Request Form. Requests may take up to 2 weeks for completion. The Title IX Office will reach out to you via phone to confirm that it has been completed. 

Chosen Name Change Process and Information

Chosen Name Change is different then Legal Name Change. If you have had your name changed through the court system please contact the Registrar located in the Records Office. 

Chosen Name Change Request Process:

  • Request is submitted through the submission of the form.
  • Title IX is notified of request.
  • Request is processed, this can take up to 2 weeks to complete.
  • Once complete, a representative from the Title IX office will contact the requestor to notify of completion.

Important Information:

  • Chosen Name will only be used on the following:
    • Class roster
    • CSC Online Learning Platforms
    • CSC Identification Card
    • CSC Eaglemail address
  • Chosen Name will not be used on the following:
    • Legal college records including:
      • Unofficial and official transcripts
      • Diplomas or Certifications
      • Paychecks
      • Payroll Records
      • Enrollment Verfication
      • Medical Records
      • Financial Aid Documentation
      • Media Releases
      • Housing Contracts
      • All other records which require use of legal name
